COVID-19: the need for ECG registration for patients

COVID-19 patients with cardiovascular disease or who are at high cardiovascular risk are the most vulnerable to multiple organ failure including myocarditis and rhythm disturbance.

Previous outbreaks of COVID-19 (SARS-CoV infection) most often caused such cardiovascular complications as hypotension, myocarditis, arrhythmia, sudden cardiac death.

Standard electrocardiography (ECG) is a recommended diagnostic test for all COVID-19 affected patients. This study does not provide any specific information; however it is known that besides decompensation of chronic concurrent diseases, virus infection and pneumonia increase the risk of rhythm disturbance and acute coronary syndrome development timely identification of which is very important for prognosis. Moreover, some changes in ECG (for example, QT interval prolongation) require close attention when evaluating cardiac toxicity of antibiotics (respiratory fluoroquinolones, macrolides), antimalarial drugs used in etiotropic therapy for treatment of COVID-19.

Inpatient and outpatient protocol implying the use of a portable ECG recorder supporting Bluetooth and a smartphone/tablet is now considered optimal for ECG monitoring.
